Failure of migrations (and this is rarely due to technology).
After repeated failures, the pattern of failure becomes predictable.
Unfinished environment assessments, which can make you unaware of everything you need to know until something breaks during the cutover. Unclear dependency mappings – when a service goes down and Service B depends on it, you may discover that Service A was dependent on it all along. Missed or rushed validation processes – which appear completed at cutover but falter afterward.
Poor stakeholder communications — Businesses only discover the impact after it has occurred and not before. Tool-first thinking: Relying on a platform that has been established to compensate for plans that were never realized
All of these issues are related to planning and discipline.
Migration failures can be predicted, but so can their success!
